Prescription Refills:

Medication refills are all done electronically now. Pllease contact your pharmacy for refills and allow our office 5-7 business days to complete refill request. 

Referrals:

Please obtain a referral prior to your appointment with our office if required by your insurance company. If referral is required and not obtained by your scheduled appointment day the appointment will be rescheduled. We apologize for any inconvenience this may cause.


Copays/Deductibles:

If your insurance company requires a copay or deductible, these fees are your responsibility. Copays are due when at check in when you have an appointment to see the doctor.

Contacting Our Office:

Our office staff is available for phone calls from 9am to 5:00pm from Monday to Thursday. Calls before and after hours will be handled by a 24hr answering service for medical emergencies. Calls regarding appointments, prescriptions and billing will be promptly answered during office hours.

Financial Concerns:
For your convenience we will bill your insurance company for you, but remember that your insurance coverage is a contract between you and the insurance company. We participate in most insurance plans - Medicare, Blue Cross, Independent Health, Choice Care etc.

Cancellations:
Rescheduling and cancellations will be charged a standard office visit fee if not received 24 hours in advance. This fee will not be paid by your insurance company.
