Prescription Refills: When calling in a medication refill, please have your pharmacy phone number available. Please give our office five business days to call in your refills. Referrals: If you belong to an insurance company that requires a referral when you see a specialist, it is your responsibility to bring a valid referral when you come for your visit. The doctors will not be able to see you without a referral. You will be asked to reschedule your appointment. We are sorry for the inconvenience but this is dictated by your insurance company. Copays/Deductibles: If your insurance company requires a copay or deductible, these fees are your responsibility. Copays are due when you check in to see the doctor. Contacting Our Office: Our office staff is available for phone calls from 9am to 4:00pm from Monday to Friday. Calls before and after hours will be handled by a 24hr answering service for medical emergencies. Calls regarding appointments, prescriptions and billing will be promptly answered during office hours. Financial Concerns: For your convenience we will bill your insurance company for you, but remember that your insurance coverage is a contract between you and the insurance company. We participate in most insurance plans - Medicare, Blue Cross, Independent Health, Choice Care etc. Cancellations: Rescheduling and cancellations will be charged a standard office visit fee if not received 24 hours in advance. This fee will not be paid by your insurance company.
